Module 3 – Satisfaction Index

[/vc_column_text][/vc_column][/vc_row][vc_row][vc_column][vc_column_text]Remember to fill in the ‘Pt’ column with the reference letter of the part they have completed, where applicable, e.g. Work ‘A’ or Family ‘C’.

Transfer the scores, i.e. 4, 2, 0, not the letters (YPN), in to the boxes under the question numbers.

Transfer the totals you calculated on the questionnaire for each part.

Remember, when adding up the total satisfaction scores, where two or more parts are completed for an area, e.g. Family A and Family B, only include one of them (usually the main role) in the overall total. If you do not do this, you will invalidate the total Satisfaction score since the potential score would then be over 100.


Frustration Index and Outlook Index

The scores transfer straight on to the front sheet from the questionnaire.

Top Box
